Making Forms

Like any content you need to consider your audience and plan carefully when making a form. The stakes are high here because you want the user to give you information, not just read your content. Keep in mind you want the information you are gathering to be complete and correct! The basic rules of good web content apply, clear language and keep it brief so that the user stays engaged and provides accurate answers.

After determining your content, properly building a form is some of the trickiest HTML you will write. Fortunately we now have access to a form building and handling tool called Cognito. Having knowledge of how forms are constructed and accessibility requirements is still key, this service just makes the building and hosting much faster.

You must follow the golden rules!

  1. You can only collect public information with Cognito or Google
  2. Forms created with Cognito or Google are not password protected.
    Use the Qualtrics survey tool if you need to assign passwords.
  3. You cannot use Cognito or Google to collect credit card numbers, money, or private information! See above.

What about Google forms?

The people responsible for reviewing web accessibility at the University recently approved Google forms as meeting accessibility requirements so these are a good option if you need a form quickly and do not need it to display as part of your site, you need to link out to the form.

Keep in mind forms Google forms are tied to the ID of the person that created the form. Consider how long the form will be used and who else should have access in case changes need to be made in your absence. Also note, Google forms are not secure! You must not use Google forms to collect credit card numbers, money, or any type of private information!

By using a few HTML elements properly such as labels, fieldsets, and legends, you can make a form that is accessible. Applications like Cognito and Dreamweaver will put most of these in for you, but knowing what they are is helpful and will elevate you to rock-star status.
